Patient Forms

Upon entering our office, our staff will welcome you as a member of our family. We will request that you complete our patient forms if you have not completed them in the comfort of your own home. This paperwork provides us with your health history and information on your condition. Rest assured that nothing will be done in our office without your consent.



Consultation

Next, you will have a consultation with Dr. DeFilippis to discuss any health-related problems, concerns, and potential treatment options. This initial visit is designed for Dr. DeFilippis to learn more about you, your health goals expectations, and any health issues or conditions you may have. This will help us to determine how chiropractic care can meet your goals.>

Examination

After your consultation, Dr. DeFilippis will perform a complete chiropractic examination testing the function of your nerve system. Other standard neurological, orthopedic, postural, and physical tests will be performed as well.



X-Ray Studies

Depending on your specific condition, we may take X-rays as well. X-rays help us in finding the cause of most problems, assist us to develop the most effective treatment plan for you, and alert us of any serious spinal conditions. Dr. DeFilippis will then thoroughly study and analyze your X-rays and exam findings and will go over all of the results with you on your next appointment. Most people are amazed once they see their X-rays up close. In some cases, they can even identify their misalignment and degeneration themselves. >



Report of Findings

At this time, Dr. DeFilippis will review your history, exam and imaging. If any special testing is required it will be ordered. You will then be invited back for a report of findings to discuss the cause of your health concerns and then discuss a treatment plan. The emphasis is to have your questions answered.
